For assistance with Registration,

To register your child for the 2017 season of Football or Cheer, please first complete the registration process online. Once that is complete, the following paperwork will be required. All can be found on the website after you've registered online:
1. Participation and ID Card
2. Waiver and Release
3. Concussion Acknowledgement
4. Resume Participation Medical Clearance
5. Emergency Medical Treatment
6. Image Release
7. Medical Clearance*
8. Code of Conduct
9. Absentee Form
*If your player has had a physical dated 1/1/2017 or later, you can use the doctor's form from that physical in lieu of the Medical Clearance form.
In addition to the forms listed on the website, you will also need to provide the report card from your child's last completed school year as well as an original birth certificate with a raised seal (birth certificate required for new players only).
